Patient Care Teams

Welkin supports a team approach to care by enabling care team member assignment to every patient. Notifications and Tasks will be directed to care team members, when there are important events that occur for a patient in the Welkin Care Portal. All Care Team members can communicate with the patient in the patient’s Communication Center, and can see all interactions between the Care Team and the patient.
